We keep the signup simple. We collect your email address and, if you choose to provide it, your first name.

Why we collect it

We use this information to provide the free appointment prep page and send occasional emails about new printables and organization tips, if you have agreed to receive them.

Who processes your information

Signups are stored in the project’s database. No mailing service is connected yet, and no email is sent automatically. If a mailing service is connected later, it will process your email only to send the emails you agreed to receive. We do not collect health information through this website.

Along with your email and optional first name, we record the time and version of your consent, campaign labels from the signup link, and a referrer page without its query string or fragment. We do not save IP addresses in the signup database or application logs.

Your choices

When emails are sent, they will include an unsubscribe link. To ask us to delete your signup information, contact [Contact email to be added before publishing].
